Mark Golledge, care and health digital lead at the Local Government Association, discusses its role in supporting every council on the 'one patient journey', and the need to look at the system as a whole to prevent people going to hospital and their care needs escalating in the first place.

He says that a digital assessment showed 80% of councils are using technology to help people stay independent in the homes - and outlines the success at Hampshire. He also welcomes the new funding from NHS Digital and outlines the work under way today.
